How we collect, use and protect information on asgfoot.com.

This website, asgfoot.com, is operated by American Surgeons Group Inc (ASG Foot & Ankle Clinic), a podiatry practice with offices in Homewood, South Chicago Heights and Mokena, Illinois. This policy explains what we collect through the website and what we do with it.
It covers the website only. Information you give us in the office, over the phone or through your medical record is governed by our HIPAA Notice of Privacy Practices, which you can request at any office.
Information you give us
When you request an appointment, request a callback, submit a wound care or contact form, download a patient guide, or subscribe to our blog newsletter, we collect what you type into that form. That typically includes your name, phone number, email address, and in some cases the reason for your visit or a description of your symptoms.
Information collected automatically
Like most websites, we automatically receive standard technical information when you visit: IP address, browser and device type, operating system, referring page, the pages you view, and the approximate city or region derived from your IP address. This is collected through cookies and similar technologies.
Calls
Some pages display a phone number that is dynamically swapped so we can tell which advertising source produced a call. Calls to our offices may be answered by an automated assistant and may be recorded or transcribed for scheduling and quality purposes.
We do not sell patient information. We do not share information you submit through a patient form with advertisers.
Any information you submit through an appointment, callback or wound care form that relates to your health or to becoming a patient is treated as protected health information under HIPAA. It is stored in our secure systems, shared only with the staff and service providers who need it to schedule and provide your care, and is never passed to an advertising network.
Display advertising on this site runs on our educational blog articles only. Those article pages contain no patient intake forms, so the advertising vendors described below never receive information about you as a patient.

These tools tell us about traffic patterns. We do not use them to build advertising profiles of patients.
Our blog articles at asgfoot.com/post/ carry third party display advertising. Our office, location, doctor, treatment and appointment pages do not.
We use Google AdSense to serve those ads. Google and its partners use cookies to serve ads based on your prior visits to this and other websites. Google's use of advertising cookies lets it and its partners serve ads to you based on your visit to our site or other sites on the internet.
You can turn off personalized advertising at Google Ads Settings. You can opt out of personalized advertising from many other vendors at aboutads.info or, in Europe, youronlinechoices.eu. You can also block or delete cookies in your browser settings, though parts of the site may not work as well.
Advertising on this site is not an endorsement. Ads are selected automatically by the advertising network, not chosen or reviewed by our physicians.

We do not sell personal information for money. Delivering interest based ads on our blog pages may be treated as sharing under California law. We keep information for as long as needed for the purpose it was collected and for as long as medical, tax and other records laws require. We use administrative, technical and physical safeguards to protect it. No method of transmission over the internet is completely secure, so please do not send detailed medical information through a web form or email if you would rather call.
We may update this policy as the site changes. The effective date at the top will tell you when it was last revised. Continued use of the site after an update means you accept the revised policy.
